Q: How would this work in an enterprise, every time a user wants to add an outside user (non domain) he has to contact the Application admin?
A: You can have one master admin and unlimited number of users with admin rights to use the DRM management. Visitors to your web site can sign up and download the activation tool, but they still need to wait until admin approves the account.
Or admin (and other entitled users) can add the email addresses to the database and send out an email with an introduction and invitation to complete registration.
If they are not in the database and approved by admin then they cannot open the documents.

Q: Can a default set of policies be applied to all documents created, eg: by default all documents will only be viewable by users within the organization?
A: Yes, access rights can be assigned per individual document, to an individual user or applied generally to a whole group.
